Purpose
The goal of this assignment is for students to do a partial community assessment and diagnose one area of need for health promotion in relation to a specific demographic.
Instructions for the Assignment
The report will begin with a description of your chosen group, as well as a rationale for the relevance of the health assessment and demographic information to put this work in context.
After that, students will undertake an evaluation using either the Population Health Promotion Model or the Canadian Community as Partner Model.
Before presenting either three determinants of health or three segments from the community assessment wheel, the student will provide a rationale for why the selected model was chosen. The data from the assessment will be analyzed.
The results of the assessment will be used to make a community diagnosis. The framework for your health promotion program in Assignment 4 will be shaped by this diagnosis.
Students will complete the following tasks in Assignment 3:
Describe the population/group you’ve chosen and explain why you chose it. Include demographic and epidemiological data that is relevant. Describe the methods of evaluation that were used:
Choose a Population Health Promotion Model, explain why you chose it, choose three health determinants, and perform an assessment in respect to the group. OR Provide a reason for your choice of the Canadian Community-as-Partner Model, select three segments from the community assessment wheel, and complete an assessment in connection to the group.
Analyze the community’s assessment data using the community assessment approach you’ve chosen. Analyze the assessment data in a clear manner. You have the ability to develop different community diagnoses based on your analysis.
Make a single, well-written communal diagnosis. Determine whether the diagnosis is one of wellness or one of deficit. In Assignment 4, your work will be framed by the community diagnosis.
Provide a reason for the community diagnosis based on the study of the community’s assessment data. Criteria for Evaluation Each of the assignment’s requirements is addressed. Throughout, the ability to analyze, evaluate, create, and engage in critical inquiry is demonstrated.
